We do not accept prescription requests by telephone.

In Person	You can attend the surgery to make a prescription request, or drop off a request through the letterbox
By Post	Repeat prescriptions can be requested using the white repeat slip which is attached to your previous prescription.  Tick the items required and either post it, fax it or place in the letter box at the practice entrance.  Alternatively use a piece of paper to write your name, address, date of birth and medication needed.
